Treatment Plan

Initial Management

– Pain control

– Supportive weight bearing (Crutches if required)

– Reduction of swelling via NSAIDs and ICE

– Early physiotherapy to help regain pain free range of motion and guide wean off crutches

– Referral to an Orthopedic Surgery

 

Key Points:

  • In the vast majority of ACL cases the diagnosis is reached based on history and physical exam.  An MRI is usually not required.  To look for concurrent injury that may impact operative planning the orthopedic surgeon may order additional investigations.

  • General Approach to MSK Management = “THE GAME PLAN(s)”

    • Therapy

    • Home Exercise

    • Gait aids (canes, crutches)

    • Adaptive équipement (bracing)

    • Modalities (ice, heat, TENS, etc)

    • Education to patient (what can they expect?, prognosis)

    • Pain meds (Simple analgesics like Acetaminophen)

    • Local treatments (at site of injury like injections and topicals)

    • Anti-inflammatories (NSAIDs oral or topical)

    • Neuropathic pain meds/Narcotics

    • (s)urgical referral (if appropriate)
