What is the evidence for radiation for Dupuytren’s contracture?

Summary

Patient Population:

  • Patients with Dupuytren’s contracture
  • 1 randomized controlled trial and 5 retrospective cohort studies
  • The mean age of participants was 58.5 years. The ratio of males to females was 1.6:1.

Intervention:

  • Radiotherapy (total dose range between 21 Gy to <42 Gy)

Comparison:

  • Studies either did not have a comparison group or had a comparison group with a different radiation dose.

Outcome:

Disease regression (reduction in number and consistency of cords or nodules, and reduction in extension deficit).

  • Three studies reported this outcome [1-3], with a range of 0-56%.

Disease stability (neither regression or progression of disease).

  • Three studies reported this outcome [1-3], with a range of 37-98%.

Disease progression.

  • Three studies reported this outcome [1-3], with a range of 2-20%.

Proportion of patients requiring surgery following disease progression after radiotherapy

  • 3.1-10%.

Symptomatic improvement:

  • ranged between 66-87% following radiotherapy.

Complications

  • Erythema, skin dryness, and desquamation occurred in 20-43%.
  • No study reported radiotherapy-induced malignancy.
